The Village of Hawthorn Woods has provided the two forms below as a convenience to those who wish to request records through the Illinois Freedom of Information Act. Records can also be requested in person at Village Hall or via fax. Those with questions are encouraged to call Village Hall at 847-438-5500 and ask to speak with one of the Village's FOIA Officers. FOIA Requests can also be mailed to one of the Village's FOIA officers at 2 Lagoon Drive, Hawthorn Woods, IL 60047.

The forms below are automatically sent to them.
Per the FOIA Act, there is no fee for the first 50 pages of black and white, letter or legal sized copies. After 50 pages of black and white, letter or legal, the Village charges $.15 per page. If the records requested are in color and you would like color copies or you would like copies larger than letter or legal, the Village will charge the cost of producing those records.
There is a charge of $1.00 to certify a record. If the record is maintained in electronic format, the Village will provide that to the requestor at no cost.
For commercial requests, a public body may charge up to $10 for each hour spent by personnel in searching for and retrieving a requested record, but no fees shall be charged for the first 8 hours spent by personnel in searching for or retrieving a requested record.
For commercial requests, the response time is 21 working days, per the FOIA Act. For non-commercial requests, the response time is five business days.
